Operator: Prime Hotshots Volleyball Club (PHVC).
Period and activities: October 1, 2026 through September 30, 2027, for PHVC-organized indoor, beach and grass volleyball scrimmages, tournaments, leagues and training activities, subject to event eligibility and any additional event-specific documents.
Risks: Volleyball includes foreseeable and unexpected risks, including falls, collisions, contact with balls, equipment or other participants, strains, sprains, fractures, concussion, serious injury and, rarely, death. Facilities and playing surfaces may have hazards. Participation is voluntary.
Safety: Players agree to comply with event rules, reasonable safety directions and venue requirements; report unsafe conditions and injuries; and refrain from play when medically unfit. Organizers may restrict participation for safety or eligibility reasons.
Medical response: In an emergency, organizers may contact emergency services and the listed emergency contact. This does not guarantee medical care or transfer medical costs to the club.
Legal status: This agreement documents participation risks, safety responsibilities, and consent. It does not release negligence claims or guarantee insurance coverage. Parent or guardian consent authorizes a minor’s participation but does not release the minor’s legal claims.
PHVC may photograph or film club activities. If you consent, PHVC may use identifiable images/video of the player on its website, Facebook, Instagram, YouTube, event recaps, posters and other club promotional materials. Content shared publicly may be copied by others. You may contact the club to withdraw consent for future use; previously distributed or third-party copies may not be retrievable. Saying no does not prevent participation. PHVC will use reasonable measures to respect non-consent.
Adults sign for themselves. For players under 19, the parent or legal guardian must sign. The signature must match the relevant name above. Submission does not independently verify identity.
